This role will involve providing a range of personal, social, emotional and practical care tasks as required for the well-being of clients living in their own homes within the community and within the package of person centred care to enable clients to exercise control over their own lives by maximising their independence.

Supporting a group of female residents with learning disabilities and down syndrome in a supported living setting, a block of flats with a communal lounge and kitchen.

The duties and responsibilities of the role are as follows:
- Observe each clients changing needs and reporting, on a regular basis, to the Domiciliary Care Manager.
- Providing personal care to clients.
- Involving clients in social activities such as shopping, maintaining contact with family and friends and escort duties which may include places such as banks, doctors surgeries or medical appointments.
- Undertake essential domestic duties assisting clients to care for their homes, whilst encouraging clients to retain their independence.
- Making medical appointments for the clients.
- Assisting clients in paying household bills and rent enabling them to retain their independence.

In order to facilitate meeting the needs of the clients it is an essential requirement of the job that the candidate will be required to work flexible and unsocial hours. This includes evenings, weekend and bank holidays.

Previous experience of giving personal care to adults with disabilities is essential.

The post holder will be required to undertake a fully enhanced DBS check at their own cost.

Domiciliary Care Coordinator - coordinator / 31/01

Previous experience of domiciliary care coordinating is essential - those with NVQ/Diploma Level 3 (or above) in Health and Social Care (or equivalent) are particularly desirable.

You will be working with an established team, which coordinates care workers undertaking approximately 2500 hours of care per week.

This role will involve managing rotas, liaising with social services and the next-of-kin of service users, rostering and allocating hours to care workers, using IT systems (for which full training will be provided), and participating in the on-call on a rotatory basis.

Candidates with good organisational skills who are honest and outgoing are highly desired. Familiarisation with Carefree and/or CM2000 would be a plus.

Role:
*To work with the best interest of the company and to successfully co-ordinate homecare packages
*Responsible for contributing to branch target and for ensuring the development of the business through existing and new clients and service users
*To maintain high quality standards, systems and procedures set by the company and in line with CQC Compliances and Health & Social Care Act 2008 and contract compliance as applicable.
Responsibilities:
*In conjunction with the Home Care team ensure that the branch is compliant
*To operate and comply within Health & Social Care Act 2008 and to adhere to CQC regulations
*To adhere to branch systems and procedures in accordance with company policy, auditing requirements
*To attend regular updates; daily, weekly and monthly, branch focus meetings and business reviews
*To operate and perform within branch / company KPIs
*To co-ordinate issues and problems that may arise with clients and candidates and investigate / resolve where required seeing through to an appropriate conclusion, using iTHC complaints and incidents policies
*To support effectively with candidate attraction strategies, ensuring consistent flow of good quality candidates
*To interview and place candidates ensuring that their have met auditing clearance standards as appropriate
*To ensure all communication logs, MAR Sheets and charts are returned to branch on a monthly basis and audited in-line with requirements
*Complete supervisions, work place assessments and appraisals for care workers meeting KPI's
*Complete Risk Assessments as per CQC / contract specifications
*To deliver Home Care Inductions to all new starters as required
*To carry out on-call duties on a rota basis with team
*To assist with business development with existing clients
*To manage your individual desk and workload
*To be knowledgeable and familiar with current and new legislative changes within recruitment and domiciliary care ensuring iTHC compliance at all times
To meet and maintain targets in relation to hours v income, quality assurances, KPI's, CQC compliances and action plans
